Typical Morton’s neuroma include getting sharp pains in the foot between the base of the third and fourth toes, often shooting into the fourth toe as well. The pain is noticed when walking or running at specific times. It is a sharp, “nerve” pain, which goes away quickly, but occurs frequently.
The plantar digital nerve runs along the middle of the plantar (bottom) surface of the foot and through the gap between the third and fourth metatarsal heads. The metatarsals are the long bones of the foot with connect your toes to the bones of the ankle joint. The ligaments connecting the two tarsal heads develop adhesions and fibrosis over time and cause a trapped nerve in the fascia as it runs underneath the ligaments. This entrapment then leads to Morton’s neuroma symptoms.

Our orthopaedic massage therapist’s first step is to assess the client to establish the likely condition. There are simple tests for Morton’s neuroma including squeezing of the metatarsal heads to reproduce the pain, and pressing directly on the nerve as it runs between the metatarsal heads.
Once the likely condition is confirmed, treatment includes releasing of all the flexor muscles and tendons of the foot as long as the plantar fascii. Using deep cross fibre massage techniques, the hallucus adductor muscles are then released, especially the transverse group. These muscles squeeze the toes together and contribute to the impingement. The next site of attention is the transverse metatarsal ligaments, which are released using rapid cross fibre frictions. Finally, the nerve itself is mobilized to free it up from adhesions with the surrounding tissue, and to help it sit in the right position.
Depending on the length the client has had this condition, results are often noticed within 5-6 session of the above treatment.
